FINANCIAL INFORMATION:

     We will be pleased to discuss your fees as best we can determine them and methods of payment including short-term, budget arrangements. Whereas we will be happy to help you with your insurance claim form and in some cases mail it to your carrier for you, we respectfully ask you to settle your financial responsibility with us privately and recoup your entitlement from your insurance company directly realizing that the entitlement will not cover the fee entirely. Doctor Hebert wishes to keep his relationship with you uncomplicated by transactions with your insurance company. We offer credit card services, and frankly, also suggest you consider arrangements with a preferred banker when credit becomes necessary. We will be able to help you with any of these needs. All we ask is that you notify us of any special concerns in this regard in advance. We request payment in full at the completion of treatment which may occur at the FIRST visit.

     Fees will vary with the difficulty or complexity of treatment and will be quoted in advance. If this is not done, please do not hesitate to inquire about the fee.

     Please make every effort to keep your scheduled appointments and we shall do the same; however, patients in pain will receive priority over others, and therefore the schedule may occasionally be altered without our having the opportunity to notify you in advance.  A reservation fees will be charged when twenty-four hours notice of your inability to be present is not given us.
